As you may have noticed, some requirements need parental and counselor permission.
I had drawn up some generic permission slips and passed them out at the last meeting. You can use those or just make up something yourself, with your parents/guardians giving permission for whichever specific requirement you are doing at that time... something like this:

I _____________________, the parent/guardian of _________________________, a member of Troop 258 and scout in the Citizenship in the Community Merit Badge Class,
Do hereby give permission for my son to attend/watch the movie/volunteer for ___________________________
Parent/Guardian Signature: __________________________________
Couselor Signature: ____________________________________
Date: _______________________________
